Nottingham City OwnHealth

Nottingham City OwnHealth® is a partnership between NHS Nottingham City, NHS Direct and Pfizer Health Solutions. 

The service, which began in July 2009, provides a telephone based, proactive care management service for up to 2,000 patients in the East Midlands with diabetes and/or chronic obstructive pulmonary disorder (COPD).

The aim of the service is to improve health outcomes and reduce health inequalities by working with patients whose quality and length of life are adversely affected by one or more long-term conditions.  The service addresses patients’ health and social care needs, supporting them to gain better control of their condition and make positive lifestyle changes.

Reduced need to go to hospital

Nottingham City OwnHealth®, which has initially been commissioned for three years, is based on the OwnHealth® model currently delivered for NHS Birmingham East and North in the West Midlands region.

Evaluation of the Birmingham OwnHealth® scheme over a two year period has demonstrated that 73% of members felt more confident about caring for themselves; of those who made hospital visits, two out of five stated that Birmingham OwnHealth® reduced their need to go to hospital; 67% of members were exercising regularly at the end of Year 2 compared with 39% at the time of their initial assessment; 90% were non-smokers, had stopped or were actively trying to stop smoking at the end of Year 2 compared with 73% at the time of their baseline assessment.
